When a heart wants someone and feels this deeply for someone, they only see them and only want them, and if there is someone else causght their eyes, then. . they really don't feel that way, but I can really tell that you are all about this person, who makes you feel alive. . and you wanted to thank them with this poem, it's a very sweet gesture :)

I could break and torn into pieces,
But my heart kept telling me,

hmm. . this part really bugs me, you say "i could break and torn into pieces" torn is past tense.. and break is.. well not. . maybe you mean " I could break and tear into pieces. . .but overall it's a lovely poem :)
