My name is Suicide
I live in your veins
I sneak into your head
And drive you insane

^^ I like that you chose this stanza to start, because suicide is the ultimate ending when you know these thoughts the way that you've described them here. I really like the way you worded this one.

My name is Death
I bring great sadness
Many confuse me
As a sound of madness

^^ This one is unique, it has a nice ring to it, but I had to reread it a good bit to understand it was more simple than i originally thought.

My name is Hate
I live in your heart
I bring you down
And rip you apart

^^ I like this one, because hate is something we all understand, we all experience even if there has not been a suicidal period or depression.

You'll never know me
You don't deserve
The happiness I bring

^^I think this one should have been moved to before Life, it feels out of place here. However it is extremely well done.

The only home you've ever known

^^ This one is pretty good, but seems a little awkward as far as tempo is concerned.

I make you feel hideous
And so untrustworthy

^^ another one anyone can relate to. And very good job with taking something so common and making it as strong as you did here.

I think it should be 'flood of sadness' or some other dark adjective, but not great.
Then, this stanza should be the second last stanza.
