That reads like an urban legend but it was real, a human error with tragic results

It was not a surgeon but a medical technician who injected the wrong solution. The 69-year-old woman was being prepared for what is a very serious surgery to repair a brain aneurysm. The hospital has admitted that the failure was due to the lack of systematic procedures to prevent fluids like these from being mixed up.
