I don't exactly know who this poem is about but who ever it is about I'm proud of her :) Because it is such a struggle to have to go through these kind of things and it's so hard to cope with everything.
You really created a scene in the beginning of a poem I could imagine her "crawling on the floor" and everything else that you said your choice of words really helped this.
I love the last line as well because it creates a powerful impact on the reader saying that no matter what situation you are in you can change your life around. :)
