Good job with this poem. It seems sort of unique, well written, and although it is kinda-sorta a cliche topic it was written with a well developed flow.

Love the rhyming, repetition and questions. And the pause in the last line was super-effective. My favourite stanza was:

"If I vanished tomorrow and you didn' t know why,
Would you try to find answers?
Or just accept the good-bye?"
